Vasculitis of the lower extremities and its treatment

Vasculitides is a group of syndromes associated with inflammation of blood vessels. They often develop on the background of other pathologies, but it is a separate disease. Can affect vessels of any caliber: small, medium, large. Inflammation can develop in the blood vessels of certain organs: kidney, brain, skin. Vasculitis accompany diffuse connective tissue diseases (rheumatoid arthritis, systemic lupus lupus, scleroderma).

The causes of pathology is unknown. It is assumed the influence of genetic factors in combination with external causes and infectious diseases caused by Staphylococcus aureus and hepatitis b virus. The main role of the deposition in the vessel walls of immune complexes.

The symptoms are varied. Cutaneous manifestations, particularly the rash on the legs, the most common. For example, hemorrhagic vasculitis characteristic spots on the legs and body but to skin syndrome often joins articular, abdominal, renal. Rash on lower extremities are found in other types of pathology.

Allergic vasculitis is divided into superficial and deep. The first category includes the following types:

  • hemorrhagic vasculitis (Schönlein-Henoch);
  • nodular necrotizing vasculitis (Werther-Dominga);
  • allergic arterioles Router;

Elements of the rash when knotty vasculitis represent subcutaneous nodes on the shins, first red, then bluish in color, painful and tight. Are groups formed after the collapse of long nonhealing ulcers with ragged irregular edges and a purple halo around. After healing may remain scars.

In hemorrhagic, or Henoch-Schonlein purpura on legs, arms, buttocks and torso abundant rash appears in the form of symmetrical papular-hemorrhagic elements in red with a diameter of 3 to 10 mm.

When leukocytoclastic vasculitis on the shins and other parts of the formed plaques, pustules, bullae, ulcers.

Hemosiderosis, or gemorragicescom-pigmented dermatoses is often manifested by rashes on the legs, the rash then moves to the thighs, feet, other parts of the body, the face. The elements are pigmented plaques of different sizes with scaling and slight itching.

Treatment

Patients with vasculitis can assign:

  • corticosteroids;
  • non-steroidalanti-inflammatory drugs;
  • zitostaticski;
  • plasmapheresis;
  • hemosorption;
  • antibiotics.

The treatment is carried out in three stages:

  1. A short course of aggressive therapy.
  2. Long-term use of immunosuppressants to maintain remission (up to two years).
  3. Therapy recurrence.
