Coronary stenting (vessels of the heart)

Surgery established technique, not only coronary stenting (vessels of the heart), but also stent implantation in the carotid artery to eliminate the signs of brain ischemia in the femoral — for the treatment of atherosclerotic changes of the legs, the abdominal aorta and iliac when there are significant signs of atherosclerotic lesions.

What is a "stent", variety

A stent is a mesh tube is lightweight, durable enough to ensure the frame of the artery for a long time. Stents are made of alloys of metal (usually cobalt) in accordance with high technology. There are many types. They differ in sizes, the grid structure, the nature of the coating.

The selection of patients with ischemic heart disease for surgical treatment engaged a consultant cardiac surgeon. In the clinic at the place of residence of the patient is the minimum necessary examination, including all mandatory blood and urine tests to determine the functioning of the internal organs, lipogram (total cholesterol and fractions), blood clotting. Electrocardiography allows to specify the areas of myocardial injury after infarction, the prevalence and localization process. Ultrasound of the heart clearly on the pictures shows the functioning of all divisions of the Atria and ventricles.

The inpatient Department carefully carried out the angiography. This procedure involves intravascular injection of a contrast agent and a series of x-rays undertaken as the filling of the vascular bed. The identification of the most affected branches, their localization and the degree of narrowing.

Intravascular ultrasound examination helps to evaluate the possibility of the artery from the inside.

The survey allows the angiosurgeon to establish the exact place of the alleged introduction of the stent, to identify possible contraindications to surgery.

Indications for surgery:

  • severe frequent attacks of angina, determined by the cardiologist as a pre-stroke condition;
  • support coronary artery shunt (bypass surgery is to install an artificial blood flow to bypass the blocked vessel), which has a tendency to decline within ten years;
  • for health, with severe transmural myocardial infarction.

Contraindications

The impossibility of the insertion of a stent is determined at the time of the survey.

  • Common defeat of all coronary arteries, due to which there is no specific place for stenting.
  • The diameter of the narrowed artery is less than three mm.
  • Reduced blood clotting.
  • Dysfunction of kidneys, liver, respiratory failure.
  • Allergic response of patients to iodine preparations.

The advantage of stenting over otheroperations:

  • low invasiveness of the technique — no need to crack the chest;
  • short period when the patient is in the hospital;
  • relatively low cost;
  • fast recovery, return to work, the lack of long-term disability of the patient.
